Practical tips and tricks of performing the iFR Pullback
ОглавлениеAllow a steady trace before pullback: When performing an iFR pullback assessment, a short period of time should be allowed from the moment the pullback is activated to when the pullback is started. This will enable a steady line for pullback to be displayed. If there is movement of the line before pullback has started, it is advisable to cancel the pullback and start again.
Pullback the wire without opening the pressure port: the pressure wire can be withdrawn without altering the O‐ring or opening a port.
Continuous fluoroscopy should be used when using co‐registration systems: If this is not available, a brief fluoroscopic image stores should be made when the pressure steps are noted on the pullback trace. This will aid the interpretation of small or steep changes in pressure and help the operator co‐localize the pressure changes with angiographic appearances.
Always return to the ostium: The pressure sensor should be moved to the ostium of the vessel as part of the pullback. This means that any drift can be identified. The pullback systems automatically reformat the pressure trace for small degrees of pressure‐wire drift which assists in interpreting the data collected.
