There are not enough pages to thank everyone who so generously contributed to the creation of this book. Hundreds of patient and practitioner interviews and countless individuals who let me sit in and observe their care and treatment. Researchers who so graciously shared the details of their findings. And most of all my family, friends and dogs, who cooked me a meal or went with me for a walk in the mountains just to help me get through this time-consuming process.
