Classification of Middle Ear Reconstruction: Table 1 The Ideal Prosthesis

The key attributes for an ideal prosthesis are:

1. It must be biocompatible, avoiding unwanted reactions, such as inflammation or neoplasia.

2. It should not be biodegradable, that is, losing form or stiffness with time.

3. It must transmit force efficiently, that is, not itself have high mechanical impedance, and be relatively rigid.

4. It should ideally be able to adapt to static pressure changes in the middle ear and maintain its efficiency.

5. It should be easy to place, with good surgical handling characteristics.

6. It should be easy to revise.

7. It should be stable in that it stays in position in the long and short term.

8. It should be relatively cheap and adaptable.

9. It should be compatible with imaging, for example, MRI or CT imaging, and not cause too much artefact.

It would be perhaps even better than the unreconstructed ear if it could reverse middle ear pathology by biological coatings that inhibit or reverse inflammatory processes.
