Very poor results in middle ear reconstruction are dominated by pathological variables, such as poor ventilation and scarring. Little can be done in prosthesis design to counter this, beyond adding biologically active components. In general, almost all prostheses are “good enough” mechanically to transmit forces from the TM to the remaining ossicles. Surgical placement and prosthesis design should be chosen to maximize piston-like movements, and constrain rotational movements. Increased tension has a large effect on lower frequencies, most other parameters have small effects. It is quite possible that the parameters work non-linearly in combination though, so that a combination of “small effects” reduces stapes vibrations more than the sum of the individual effects.
