In the Kashiwaba Neurosurgical Hospital PDT with Photofrin^® was performed in more than 70 cases of HGG located both in the brain and spinal cord. The whole cohort can be subdivided into 3 groups. First, patients with unresectable deep-seated malignant brain gliomas underwent interstitial photo-irradiation after stereotactic insertion of an optical fiber into the tumor. Second, for tumors located within functionally important brain structures (e.g., motor or speech cortex) or infiltrating spinal cord, for which total resection posed a high risk of serious postoperative neurological deterioration, maximal total resection under the guidance of 5-ALA-induced fluorescence was performed initially with subsequent intraoperative PDT of the residual neoplasm [9, 11, 13, 15]. Third, after apparent total resection of malignant cerebral gliomas without identifiable tumor remnants, intraoperative photo-irradiation was applied on the walls of the entire resection cavity for elimination of the infiltrating tumor cells.
