Retention of surgical instruments is not the only error in veterinary surgery. Performing the wrong procedure on the wrong patient is of real concern, particularly because veterinary patients cannot themselves confirm their own identity. Consider, for example, two orange tabby cats of roughly the same build and body weight, one of which presents for onychectomy and one for tendonectomy. These are two very different procedures, with two very different patient outcomes. Failure to accurately identify each individual may result in the correct procedure being performed on the wrong patient.

Another surgical error that the author witnessed as a veterinary student stems from failure to confirm the feline patient's sex prior to sterilization surgery. Attempting to spay a tomcat will continue to happen in clinical practice so long as clinicians and support staff put blind faith in the client's ability to sex the cat, particularly in this era of pediatric neutering.
