The Era of Decline of Stereotactic Surgery
ОглавлениеThe heyday of movement disorder surgery was rapidly eclipsed by the development of L-DOPA and the finding of more effective medical therapies for both Parkinson’s disease and other movement disorders. The training of stereotactic surgeons rapidly declined in the 1970s, and by 1980 relatively few surgeons were trained in movement disorder surgical techniques. The management of movement disorders largely passed to the hands of medical neurologists. A few individuals such as Phil Gildenberg, Pat Kelly, and Ron Tasker continued to use and advance the techniques as new imaging tools such as CT were developed. Introduction of axial plane imaging for the first time allowed accurate individual patient definition of the internal capsule, improving targeting that was still based on relationships to the intercommissural line [19]. Pooled atlases of the deep brain targets provided guidelines, but now critical structures such as the internal capsule could be seen on each individual patient.
