For nearly a century, neurological surgery has played a critical role in advancing our understanding of movement disorders and in developing related therapies to help these patients. This volume of Progress in Neurological Surgery seeks to summarize the current state of modern movement disorders management, where multiple surgical therapies now may be options for patients whose symptoms are not adequately treated with medication. Following an initial chapter that briskly takes us through the history of movement disorders surgery, excellent chapters by leaders in their fields describe the pathophysiology, functional neuroanatomy, clinical presentation, and medical management of Parkinson’s disease, dystonia, and essential tremor, including an updated overview of basal ganglia thalamocortical circuits. Leading neurosurgical practitioners then cover the spectrum of movement disorders surgery itself, from the lesioning procedures of radiofrequency ablation, stereotactic radiosurgery, and high-frequency ultrasound to the modulatory procedures of “asleep,” image-guided deep brain stimulation (DBS) and “awake,” microelectrode-guided DBS. The final group of chapters are a unique collection by experts in closed-loop DBS, drug-delivery, gene therapy and other emerging neurosurgical therapies, highlighting long-standing experimental strategies that are reaching exciting phases of clinical translation. We hope readers will find this volume to be a valuable resource for accessing the wide spectrum of concepts that currently define this dynamic field.
