Just applying the anesthesia does not make use of it. The endoscope now needs to be passed into close proximity with the vocal cords. Part of a close exam is the short physical distance of the endoscope to the vocal cords and part of a close exam is observing several cycles of functions such as breathing and also phonation. The endoscope tip can be flexed for a nearly parallel view along the length of the vocal cords.
