Our results suggest recommendations and a possible algorithm to reduce LVH in CKD patients. BP must be kept lower, and Alb must be maintained, for example, by decreasing urinary protein before initiation of dialysis. SBP must be kept lower and plasma ANP must be decreased by controlling overhydration; Hb levels must also be maintained after initiation of dialysis, but it is difficult to improve LVH in HD patients. LV structure, contraction, and compliance were well preserved in PD patients undergoing aggressive treatment based on plasma ANP and LAD measurements. It is important to treat hypertension, overhydration based on ANP, and anemia before initiation of dialysis. Our findings may offer therapeutic strategies for treating predialysis and dialysis patients.
