The Prevalence of GDM

Implementing the IADPSG criteria on the HAPO study population sets the prevalence of GDM at approximately 18%, ranging from 10 to 25%, across participating centers [38]. This point has been a major weak point in the IADPSG recommendation since it seems that the prevalence of GDM using the new criteria exceed the anticipated eventual increase in the prevalence of T2DM [39].

Several studies have reported significant increase in real-life prevalence of GDM if IADPSG criteria are implemented – escalating by 19–65% and up to 3.5-fold increase [4046]. Recently, Mayo et al. [47] compared IADPSG criteria to the Canadian Diabetes Association (CDA) criteria (universal screening of 50 g followed by a 75 g OGTT with 2 abnormal values). Because the OGTT thresholds based on the IADPSG criteria are lower than the CDA, the researchers identified a subset of 155 women with GDM according to IADPSG but not CDA criteria. Applying the IADPSG criteria to a Canadian population would increase the rate of GDM from 3.2% (or 7.3% when including impaired glucose intolerance, that is, a 75 g OGTT with a single abnormal value of the CDA criteria) to 10.3%.
