Guidelines and Training in LEMG

Until quite recently, recommendations for a standardized LEMG examination procedure were not available. Several consensus statements or clinical guidelines are now available, for instance from the European Laryngological Society (ELS), American Association of Neuromuscular & Electrodiagnostic Medicine (AANEM), or the American Academy of Otolaryngology Head and Neck Surgery Neurolaryngology Subcommittee/Neurolaryngology Study Group [13]. Herein, the technical prerequisites, and the examination procedures are now described step by step. However, LEMG is still not implemented in the clinical routine of many otolaryngologic or phoniatric departments. In a survey of laryngeal specialist in 2013 in the USA, 21% of the participants performed LEMG on greater than 50% of their VFP patients [4]. Often, LEMG recording are only performed from the TA but not from the PCA. LEMG from the PCA is much more challenging. The working group on neurolaryngology of the ELS has established a series of instructional courses at international conferences and workshops with LEMG hands-on training in combination with an interactive webpage for multimodal learning (http://www.lemg.org). It has been shown that participation in the ELS training concept improves the quality of the LEMG recordings and the frequency of PCA recordings [5].
