Left side vagal nerve stimulation is a therapeutic option for patients with drug-resistant epilepsy. Common side effects include hoarseness, obstructive sleep breathing disorders, or even permanent VFP on the left side, or only during vagal nerve stimulation, or only at rest. In such cases, LEMG can help to uncover the related reason [25]. The vagal nerve stimulation can lead to tonic spastic activity with reduced phasic modulation or enforced muscle activation in the TA, PCA, and CA appearing endoscopically like vocal fold palsy or leading to vocal fold adduction. Vagal stimulation then has to be adjusted to the findings.
