Abstract (english)

Due to the demographic developments, the need for long-term care as well as financing for long-term care is seen as a particular challenge for the future. Primary Nursing is considered as nursing delivery system in the scientific discourse for mobile long-term care and is examined for the first time in Austria regarding economical effects.
Methods:
A pilot-study with a 12-month follow-up was carried out in 2017 in Home Health Care of the Upper Austrian Red Cross. To control biases, data were weighted with propensity scores. The implementation of Primary Nursing was evaluated. A health economic evaluation provides information about the cost-effectiveness of Primary Nursing.
Results:
Primary nursing could not provide evidence of an improvement in nursing outcomes, it was shown a 1.25-fold greater benefit to the nursing service from standardized fee relative to average staff costs. With Primary Nursing, the study revealed an average annual expenditure saving of € 1,232 per patient for the payer without significant differences in nursing outcomes.
Conclusion:
In the long term, unused efficiency potentials in mobile nursing care can provide measurable added value to care through effective, health-promoting nursing interventions and targeted nursing process management, without incurring additional expenditure. Due to the lack of comparisons, further research is necessary in this setting.
