Personal Experience with Stereotactic Cryodestruction of Intracranial Gliomas

From September 1998 till September 2013, 88 patients (47 men and 41 women; mean age 43.2 years; age range 14–69 years) with supratentorial gliomas considered not suitable for aggressive resection, underwent stereotactic biopsy and cryodestruction of the tumor in the Department of Neurosurgery of the Military Medical Academy (Saint Petersburg, Russia). The neoplasms were predominantly located within the cerebral lobes (73 cases), insula (7 cases), thalamus and internal capsule (5 cases), and corpus callosum (3 cases), and typically affected functionally important cortical and subcortical structures. The mean volume of mass lesions was 44.2 cm3. The neurological symptoms and signs included seizures (60 cases), headache (54 cases), motor deficit (35 cases), speech disorders (14 cases), papilledema (13 cases), and nausea (8 cases). Overall, 73 patients had a Karnofsky Performance Scale (KPS) score ≥70. According to the current WHO classification of tumors of the central nervous system (CNS) [31] there were one oligodendroglioma, 36 diffuse astrocytomas (DA), 37 anaplastic astrocytomas (AA), and 14 GBM. Therefore, 37 tumors corresponded to WHO grade II, 37 to WHO grade III, and 14 to WHO grade IV.
