Advantages and Limitations of Stereotactic Cryodestruction of Gliomas

Our experience shows that stereotactic cryodestruction may be effectively used for management of supratentorial gliomas not suitable for aggressive resection. In such cases minimally invasive selective ablation of the most metabolically active part(s) of the neoplasm may effectively substitute open surgery directed at partial tumor removal, and provide survival benefits for the patients with tumors of the various WHO histopathological grades. As is the case for other minimally invasive procedures, stereotactic cryodestruction is repeatable and usually followed by fast postoperative recovery; thus, it allows the early initiation of adjuvant postoperative FRT and chemotherapy. However, this technique does not provide internal decompression, and, in fact, mass effect of the tumor may even increase temporarily owing to perilesional edema surrounding the area of cryoablation. Thus, the selection of appropriate candidates for this treatment is of paramount importance.
