Glycemic Control Index and Target Value Plasma Glucose Levels
ОглавлениеMeasurement of fasting plasma glucose level is not usually performed at the site of the dialysis facility, and the plasma glucose level is usually measured by a blood sample taken at the start of hemodialysis. Multiple studies comparing blood glucose levels and prognosis in dialysis patients used not fasting plasma glucose, but casual or postprandial plasma glucose levels at the start of hemodialysis [3, 4]. Therefore, the guideline of Japanese Society for Dialysis Therapy (JSDT) [2] provisionally adopts predialysis plasma glucose or casual plasma glucose level as an index of glycemic control. Generally, the start time of the hemodialysis in the morning is often between 8 and 9 o’clock, typically 1–2 h after breakfast, so the plasma glucose level at the start of hemodialysis is considered the highest value. The postprandial plasma glucose level varies depending on the time after ingestion and the dietary content, but there is a report that postprandial plasma glucose level is correlated with onset of cardiovascular disease [5]. Therefore, the postprandial plasma glucose level is also considered to be useful as an indicator of glycemic control.
