Teamwork, collaboration in multiprofessional networks, and a patient-centered approach characterize modern, integrative healthcare. This is in stark contrast to the current model of healthcare provision by GPs in single practices. The growing lack of GPs, particularly in rural areas, presents the opportunity to establish new supply structures in which many well-educated skilled employees, including salaried physicians, share the tasks of health promotion and treatment. The acute shortage of healthcare in rural areas encourages us to find solutions that are based on the principles of patient-centered medicine. The relevant opportunities arise from restructuring processes, increasing the skills of non-medical professional staff by providing additional education, improving horizontal integration within municipalities, and applying digital solutions to, inter-alia, vertically integrating primary care providers with specialists and research. This chapter presents a project currently implemented in a rural setting in the federal state of Baden-Württemberg based on the Geneva Declaration from the consensus conference of the International College of Person-Centred Medicine (ICPCM).
